PRECAUTIONS

•     It is important to read the upLift5 Instructions for Use (pdf) prior to operation for proper use, maintenance, and care.
•     The upLift5 is intended to be re-used.
•     Before use, inspect the upLift5 and packaging to verify that no damage has occurred as a result of shipping and handling. Do not use damaged products.
•     Always recharge the upLift5 in accordance with the Instructions for Use prior to use. Refer to CHARGING section.
•     Never use any other alternative power sources or instruments in conjunction with the upLift5
•     Use the upLift5 only as instructed.
•     The upLift5 is water resistant, not waterproof.
•     DO NOT submerge in water
•     DO NOT use in the shower
•     Keep USB port clean and dry
•     Wipe dry before charging after cleaning
•     The mouthpiece can be cleaned with water according to the instructions in the CLEANING section.

 CONTRAINDICATIONS

•     Do not use the upLift5 if you have conditions such as Vertigo to avoid symptoms such as headache, dizziness, and aching facial pain.
•     Do not use the upLift5 if you are pregnant.

 WARNINGS

•     Discontinue usage if you experience headache, dizziness, discomfort, or pain.
•     Do not make any modification to the upLift. Any modification may impair functionality of the device and void the warranty.

DISPOSAL

After three months of use, dispose of the mouthpiece in household waste. The actuator should be re-used with a new mouthpiece available at the upLift5 Amazon Store.

Used electrical and electronic equipment (WEEE) should not be mixed with general household waste. For proper treatment, recovery and recycling, please take this product(s) to designated collection points where it will be accepted free of charge. Disposing of this product correctly will help save valuable resources and prevent any potential negative effects on human health and the environment, which could otherwise arise from inappropriate waste handling. Please contact your local authority for further details of your nearest designated collection point/household hazardous waste collection point. Penalties may be applicable for incorrect disposal of this waste, in accordance with your national legislation.

EXPOSURE AND INTERFERENCE

•     This equipment has been tested and found to comply with the limits for a Class B digital device pursuant to Part 15 of the FCC Rules. These limits are designed to provide reasonable protection against harmful interference in a residential installation. This equipment generates, uses, and can radiate radio frequency energy and, if not installed and used in accordance with the instructions, may cause harmful interference to radio communications. However, there is no guarantee that interference will not occur in an installation. If this equipment does cause harmful interference to radio or television reception, which can be determined by turning the equipment off and on, the user is encouraged to try to correct the interference by one of the following measures: Reorient or relocate the receiving antenna. Increase the separation between the equipment and receiver. Connect the equipment into an outlet on a circuit different from that to which the receiver is connected. Consult the dealer or an experienced radio/TV technician for help.
•     Changes or modifications not expressly approved by the party responsible for compliance could void the user’s authority to operate the equipment.
•     This device complies with part 15 of the FCC Rules. Operation is subject to the following two conditions: (1) This device may not cause harmful interference, and (2) the device must accept any interference received, including interference that may cause undesired operation.

 

REPLACEMENT

If your upLift5 device or any of its components no longer works properly or requires repair, during the warranty period, Contact Us. Do not attempt to service or perform maintenance on the device. No modification of the device is allowed.

 

SERVICE LIFE

Service life of actuator is 2 years from the date of manufacture.
